================== Hazards are: • What is a Reportable Injury? : (a) Fatality; (b) A member of the public is killed or taken to hospital. (c) Fracture other than to fingers, thumbs or toes. (d) Amputation. (e) Dislocation of the shoulder, hip, knee or spine. (f) Loss of sight (temporary or permanent). (g) Chemical or hot burn to the eye or any penetrating injury to the eye. (h) Injury resulting from an electric shock or electrical burn leading to unconsciousness or requiring resuscitation or admittance to hospital for more than 24 hours. (i) Any other injury: leading to hypothermia, heat-induced illness or unconsciousness; or requiring resuscitation; or requiring admittance to hospital for more than 24 hours. (j) Unconsciousness caused by asphyxia or exposure to harmful substance or biological agent. (k) Acute illness requiring medical treatment, or loss of consciousness arising from absorption of any substance by inhalation, ingestion or through the skin. (l) Acute illness requiring medical treatment where there is a reason to believe that this resulted from exposure to a biological agent or its toxins or infected material. (m) Any other accident connected with work (including an act of physical violence) where a person is away from work, or unable to do their normal work, for more than seven days (including non-workdays). (n ) an unintended incident at work has led to someone’s possible or actual exposure to coronavirus. This must be reported as a dangerous occurrence. A worker has been diagnosed as having COVID 19 and there is reasonable evidence that it was caused by exposure at work. This must be reported as a case of disease. A worker dies as a result of occupational exposure to coronavirus The control measures are: - The CLIENT’S CONTRACTS MANAGER must, in the first instance, notify the Health & Safety Executive, without delay, normally by a telephone call, of any major injury (i.e. items (a) to (l) above. This must be followed by submission to the Health & Safety Executive of a completed Form F2508 within ten days of the Incident. In the case of item (m), this must be notified to the Health & Safety Executive by a completed Form F2508 within ten days of the incident. A copy of this form must be given to Chris Sedgeman Scaffolding Limited. How and who could be injured? Persons who have not had appropriate first aid. What is the level of risk before the control measures? = HIGH Emergency procedures: - COVID19 Will be amended to Pandemic. ======= We will require on site all welfare facilities to comply to the 2m social distancing rule. • We will require all toilet facilities cleaned on a regular basis • We will require adequate hand cleaning facilities • We will work within the NASC Site operating procedures outlined in this document • We have assessed our risk with the hierarchy of control Hierarchy of Controls – If we are not able to work whilst maintaining a two metre distance, we will consider whether the activity should continue and, if so, risk assess it using the hierarchy of controls below and against any sector-specific guidance.
